Where do I upload a new map?

Do I upload the .haz file or does it require some conversion?

You can get to the upload link from the Hacienda Create Game page.

Create a new game, and in the map selection area, there's a link that allows you to upload the map. The map won't be public until you flag it to be publically available; you can't flag it as public until after you've completed a playtesting game with it.
